The singer filed the court documents in Los Angeles after announcing the news in April that she will be separating from Simon, who she shares six-year-old son Angelo with.
Following the news of their split, a spokesperson for the couple said they remain committed to raising their son in a loving environment.
The statement read: "Adele and her partner have separated. They are committed to raising their son together lovingly. As always they ask for privacy. There will be no further comment."
The pair started dating in 2011 and welcomed Angelo together in October 2012.
