The accused, 41-year-old Babur Karamat Raja, from Sutton Coldfield, was arrested last month after a woman suffered multiple wounds to her abdomen.
Mr Raja has yet to enter a plea to two counts of attempted murder, and one of attempted child destruction.
He appeared via video link from HMP Birmingham. His victim, who cannot be named for legal reasons, was in court.
Raja was remanded in custody until a further preparatory hearing on 23 May. It is understood he is likely to face trial in August.
