The Morecambe Bay Investigation was established by Secretary of State for Health, Jeremy Hunt, in September 2013 in response to a series of maternal and neonatal deaths at the Trust between January 2004 and June 2013.
The report has revealed 20 instances of "significant or major failings of care" at Furness General Hospital, which was associated with three maternal deaths and the deaths of 16 babies. It added that different clinical care would have been expected to prevent the death of one mother and a further 11 babies.
The report also describes major failures at almost every level, including mistakes by midwives and doctors, a failure to investigate and learn from those mistakes, and repeated failures to be honest with patients and families – including the possible destruction of medical notes.
The Department of Health has said there are some actions that will be implemented immediately, following the publication, but the UK Government will examine the other recommendations in detail before providing a full response to the report.
In a statement to Parliament, Health Secretary Jeremy Hunt said: "There is no greater pain than for a parent to lose a child – and to do so knowing it was because of mistakes that we now know were covered up makes the agony even worse.
"Nothing we say or do today can take away that pain. But we can at least provide the answers to the families' questions about what happened and why, and in doing so try to prevent a similar tragedy in the future."
