Speculation has surrounded the couple's relationship for a number of months after the television personality was spotted on a number of holidays this year – without her husband or daughter.
In a statement, a spokesperson for Van Outen said: "Following continued speculation, Denise Van Outen and Lee Mead are sad to confirm they have decided to separate.
"The couple remain the best of friends, with their daughter Betsy as their number one priority. The family ask for privacy at this time."
Earlier this year, the former Big Breakfast host denied her marriage was on the rocks, but admitted it was "difficult".
Speaking to Now magazine at the time, she admitted: "To be honest with you, it is really difficult. We just haven't seen each other.
"I started on Strictly in September and we have literally been like ships passing in the night. I'm not going to lie, it is a massive strain on the relationship."
The couple first met in 2007 when Mead was a contestant on television talent show Any Dream Will Do. Van Outen was a judge on the programme. Following a whirlwind romance, the pair married in the Seychelles in 2009, and welcomed their daughter Betsy the following year.
